How often should I clean my tank?

You should take everything out of your tank and scrub it clean AT LEAST once a month. You should scrub and clean out it's water bowl once per day, as bacteria can build up in there very quickly.

How often should I change the substrate?

Every time your Boa defecates, or at least once per month. When the Boa defecates, I would recommend taking everything out and replacing it, even if you don't think any poop got on some spots of it.

Does my Boa need a bath?

Sure, if you want. Usually a Boa shouldn't stink because their skin is waterproof and not many things will stay on their skin, including blood from other animals. Soaking is a good idea to help with shedding and to encourage them to defecate often.
